Transition packages

Transition Package for Children moving schools or between school years.

The Transition Package is for families seeking support for their child transitioning from one school to another or from one year to the next.

Transitions are daunting for all children, however it can be especially scary or confusing for Neurodiverse Children – Including Autism, ADD, ADHD & Dyslexia.

As a parent, you can feel secure knowing that your child will be well prepared for their next stage.  You can be confident that some of your child’s anxiety around the transition will be relieved, which will decrease both you and your child’s stress during this challenging time.

At the end of our time together, your child will feel ready and able to progress with a great toolkit that enables them to feel more confident and better prepared for the transition.  For example, mastering the the different concepts they will need in their next stage. This can include managing and following timetables, being organised and confident, orientating themselves on a campus and managing their time and activities well.

What’s included:

  • In-home sessions with your child
  • Transition visits to the chosen School
  • Meeting with the Head of Learning Support at the new School
  • Liaison with school as required

The cost of this package is $1800. (10%off when paid upfront / payment plans can be arranged) This includes all sessions, school visits/meetings, ongoing email support, and a detailed report with recommendations moving forward (provided after the last session).

The Process:

Step 3: Commitment to an ongoing tailored plan or package outlined in your initial consultation to reach your ideal goal over a set period of time.

National Disability Insurance Scheme

Social Living Solutions is not registered with the National Disability Insurance Scheme (NDIS). However, if you choose to self-manage your package you can include my services. I will provide you with a bill, which you can then claim back through your NDIS funding package.
